Pre-Qualification Form  to Donate Eggs

Egg donors are required to meet certain criteria in order to participate. There are many reasons for the below criteria. Most are for medical reasons and we also want to present optimal egg donors to our recipient patients. We provide this criteria in order to have the best potential to be able to match a donor to a recipient.
  1. Age: minimum age of 21, maximum age of 31. There are many studies showing a decreased response to fertility medications after the age of 32. For this reason, a maximum age of 31 allows for screening to be done and at least one matched cycle to occur before turning 32.
  2. Smoking:  An egg donor must be a non-smoker for at least 3 full months before beginning screening.
  3. Height/Weight/BMI:  We require a BMI of less than 30 to participate. BMI (body mass index) is measured by using your height and weight. Women with a BMI of 30 or above, are considered obese and could potentially have a decreased response to the fertility medications. You can figure out what your BMI is by searching "BMI" on the internet. Many sites will compute it for you once you enter in your current height and weight.
  4. Adoption:  Egg donors cannot be adopted. This is due to the fact that we need a full family/medical history to provide to the recipient couple.
  5. Genetics:  Egg donors must not be carriers of genetic diseases or have a history of birth defects in their immediate family.
  6. Education:  Egg donors must have a minimum of a high school diploma or GED to participate. College educated donors are highly desired by most recipient couples.

There are no exceptions to these criteria for anonymous egg donation.
